In accordance with Texas Health and Safety Code Section 181.105, you have the right to request your health care records from your provider. To request a copy of your records, please submit a written request to your provider via the secure messaging feature. You may also request records by email our practice at Records@Altnarratives.com If you email your request, we will have to verify your identity before releasing any records.

In accordance with Texas Health and Safety Code Section 611.008, we will provide either the requested files, or notice of denial and reason by the 15th day after the request is received.

Please note that in accordance with Texas Health and Safety Code Section 611.0045, we may deny access to any portion of a record if the professional determines that release of that portion would be harmful to the patient’s physical, mental, or emotional health.

Fees for medical records are stated in our informed consent paperwork, signed by all clients at the onset of services.

In accordance with the Texas Medical Privacy Act of 2011, we are required to inform you that we store your protected health information (PHI) in an electronic health records (EHR) database. Under this law, we may not electronically disclose your PHI to any person without separate authorization from you or your legally authorized representative for each disclosure. Authorization for this disclosure may be made in writing to your provider via the secure messaging portal or via email at Records@Altnarratives.com

Please note that authorization for electronic disclosure of PHI is not required if made to:

• Another covered entity as defined by Texas Health and Safety Code Section 181.001

• In the course of your treatment goals

• In the course of collecting payment for services given

• Typical health care operations

• Performing an insurance or health maintenance organization function as described by Texas Insurance Code Section 602.053

• Or otherwise required by state or federal law or regulation
